Job Description

Make a Difference Every Day

CODAC is seeking compassionate, dependable, and team-oriented Licensed Practical Nurses (LPNs) to join our Opioid Treatment Program (OTP). This role is ideal for nurses who enjoy building patient relationships, working in a fast-paced outpatient setting, and making a direct impact in the lives of individuals recovering from substance use disorders.

  • Monitor patients for medication reactions, withdrawal symptoms, behavioral changes, and overall wellness.
  • Collaborate closely with providers, counselors, pharmacy staff, and behavioral health professionals to support patient care.
  • Participate in patient screenings, assessments, and ongoing treatment support.
  • Collect and document urine drug screen specimens and results.
  • Assist in coordinating care with pharmacies, hospitals, correctional facilities, and outside treatment agencies.
  • Maintain compliance with all medication storage, security, and controlled substance requirements.
  • Provide compassionate, respectful, and nonjudgmental care to individuals in recovery.
  • Participate in team meetings, clinical collaboration, and program support activities.
